Q:tile flooring on a cement slab?
Unless your concrete is already cracked you shouldn,t have any problem. I would make sure no ground water was allowed to accum. near the walls especially if you have no footing under your breezeway slab The heating mat. is a good idea. I would go right over the slab with the mat and porc. tile. Good luck ,and have fun

Q:Do I need plywood and cement board or just cement board?
You don't need either if the concrete is smooth & level; just properly troweled cement. If the concrete is not smooth, lay a plywood sub-floor, then cement the tile down. Cement board is used on walls for ceramic tile, when water is present.
Q:Can I ceramic tile on a subfloor and 1/2" plywood and get a long-lasting floor? or must use cement board?
Plywood, yes. OSB, no. Backerboard, no... and here's why: deflection standards. The 1 1/8 thickness you achieve by adding a plywood subfloor brings it up to acceptable level. Common misconception in the industry that backerboard adds to the structural integrity, when in reality, its primary purpose is to provide a good substrate for the thinset to bond to. However, plywood is suitable if you use the proper thinset (important). Since the house is older, I would also recommend an anti-fracture membrane atop the subfloor.

Q:What is the correct process for installing cement board/water proof barrier in a shower?
I normally install green board to the studs tape the seams and the corners and then install the wonder board over the green board. This gives a float look and a full one inch barrier. it also brings the wonder board out far enough so I can drop over the lip edge. I space the wonder board 1/8 to 1/4 from bottom I silicone seal the bottom edge to the tub/shower pan I silicone seal the outside edge to the wall and the bottom edge to the floor. I run a thin bead from the bottom of both corners maybe 12 inches up. I use proper screws ( rock on ) and plenty of them. then tile I would never use an enclosure as the wear with time and look cheap. the seams are problematic and tend to leak over time and might be the source of your problem good luck
